Senior Support & Advice
Many seniors and individuals who need support report that they face barriers to accessing services and struggle to understand the different service systems. A service system isn’t client focused if participants can’t understand, access and navigate it. Diverse communities and individuals, including people from culturally and linguistically diverse (CALD) backgrounds, need specific supports, including advocacy and advisory services to understand, access and navigate the home care service system. Timely and supported access to services is critical to prevent further decline and ensure safety.
The official name of this program is the Information Referral and Support Program, but think of it as your personalised support to understand and access the aged care service system in Australia, including how to access My Aged Care (MAC), the Government system for Aged Care.
We provide personalised support to seniors, community groups, carers and other eligible people to access up-to-date home care and support services information.
Our Information and Support Services include:
- Initial care needs identification
- Information on support services and My Aged Care
- Help to understand and access assessments and approved support services
- Advice to navigate the service system
- Referrals
- Client advocacy
- Home care advisory services
- Continence advisory services
- Advisory services for diverse groups
- Dementia advisory services
