About this service

The Assistive Technology and Home Modifications (AT-HM) scheme helps older people access equipment and home changes that make everyday tasks easier and improve safety and mobility at home.

The AT-HM scheme is part of the Support at Home program. It provides extra funding to help older people stay independent, safe and comfortable in their own homes.

This funding is separate from your regular home care budget. This means you can receive assistive equipment or home modifications without reducing the funds used for your everyday care services.

 

What can it include?

There are two types of support and you may be eligible for both types of support, based on your needs:

  • Assistive Technology – items that make daily life easier and safer, such as walkers, shower chairs, or special tools to help with memory and communication.
  • Home Modifications – changes to your home so it is easier to move around, like handrails, ramps, or bathroom adjustments.

Some equipment or bigger home changes may need advice or a prescription from a health professional (like an occupational therapist).

This funding will cover:

  • the items
  • a prescription from a health professional, if needed
  • wraparound supports to ensure the participant can use the item safely and effectively
  • administration/coordination activities.

How do I access it?

  1. Assessment – During your aged care assessment, you will also be checked for AT-HM eligibility.
  2. Decision – If you are approved, your Support Plan will show:
    • the type of help you can get, and
    • the level of funding (low, medium, or high).
  3. Share your plan with AMCS – We will help organise the equipment or home changes.
  4. Use the funding – Funding is usually available upfront for 12 months.
  5. Contribution – You may need to pay part of the cost, depending on the item or service.
