Restorative Care Pathway

About this service

The Restorative Care Pathway offers short-term allied health support to restore strength and mobility. It offers up to 16 weeks of support from nurses and/or allied health professionals (like physiotherapists or occupational therapists) to help you maintain or regain function or ability after illness or injury.

It focuses on improving your strength and daily skills, managing age-related changes, and providing guidance so you can maintain your wellbeing and remain independent for longer.

What can it include?

The Restorative Care Pathway provides:

  • Up to 16 weeks of support, with extensions to 16 weeks in some situations (for example, in regional and remote areas, or to allow culturally safe care to be delivered).
  • Coordinated allied health services, such as physiotherapy, occupational therapy, dietetics, exercise physiology or speech therapy.
  • A restorative care partner, who works with the individual to set goals, develop a plan, and organise the right services.
  • Practical strategies to build strength, improve daily function and support community connection.

 

How the Restorative Care Pathway can assist

The Restorative Care Pathway helps to:

  • Regain independence – by rebuilding strength, balance and confidence.
  • Prevent or delay decline – supporting wellbeing before higher levels of care are required.
  • Promote safe living at home – through personalised therapy and practical solutions.
  • Encourage participation – enabling older people to continue with hobbies, social activities and family life.

Funding

Each period of restorative care provides about $6,000 to support up to 16 weeks of more intensive services. This funding is separate from your ongoing Support at Home funding. If eligible, you may receive extra funding, increasing the amount available to up to $12,000 during that period.

You can access restorative care twice, as long as the periods are not back-to-back and there is at least three months between them.

How do I access it?

An aged care assessment is used to decide whether a person may benefit from short-term, focused support through the Restorative Care Pathway.

A person may be eligible if:

  • Will be able to continue living at home without ongoing Support at Home services, or without changes to their current supports
  • Short-term, intensive care from allied health and/or nursing professionals is expected to help improve a specific issue
  • They are willing to set goals and actively take part in activities to build independence

A person cannot access the Restorative Care Pathway if they:

  • Are receiving End-of-Life care
  • Have already received two units of restorative care funding in the past 12 months
  • Are receiving, or eligible for, transition care
  • Are living in permanent residential aged care
