Government funded aged care services under the Support at Home program are for seniors who meet certain conditions.
Short-term pathways are part of the Support at Home program. They provide time-limited supports and extra funding to support independence, renablement and care needs.

Learn MoreUnder Support at Home, funding is organised into eight classifications based on care needs, each with an annual budget that is reviewed quarterly to ensure support continues to match changing care requirements.
Support at Home clients receive a budget with their Support Plan, and will be assigned to one of eight care classifications. Each classification is linked to a quarterly budget and is made up of government funding and your contribution. Services Australia decides your contribution, based on your income and the services you use.
Services are grouped into three types of care. Each have different levels of government funding and the client contribution is as follows:
Clinical Supports – No client contribution
Services in the Clinical Supports category are fully funded by the government
Independence Services – Small client contribution
Services in the Independence Services category require a moderate contribution by the client.
Everyday Living Services – Higher cost
Services in the Everyday Living Services category require a higher contribution by the client. These services (like meals, cleaning, gardening) are not usually funded by the government at other life stages.

Follow these three steps and once you have a Support Plan assigned by My Aged Care, our friendly team at AMCS will work with you to provide the personalised services that you need.
Step 1: Register
Call My Aged Care on 1800 200 422 or visit their website to register.
Step 2: Assessment
An Aged Care Assessor from My Aged Care will meet with you. They will talk with you about your health, home and daily needs.
Step 3: Support Plan
Together, you and the assessor will make a Support Plan. This plan shows the services that can help you stay safe, healthy and independent in your own home.
My Aged Care is part of the Australian Government. It was established to help you find and access the right government-funded aged care services.
You can begin by calling My Aged Care at 1800 200 422, ask for an interpreting service if you need one, and explain to the agent what your needs are. After an easy registration of your personal details, contact details and needs, My Aged Care will organise an assessment appointment of your needs, at your home. An Assessor will decide if you can access the first level support (CHSP) or the Support at Home Program and once you’re assigned a referral code for services, you are able to choose a “preferred provider” to provide you with those services and that’s where we come in!

If you have successfully met the requirements to be eligible for Home Care, you will be referred for an assessment by My Aged Care. They usually meet in person and visit your home. It is during this visit that they decide which level of service you require.
